This program offers case management services aimed at helping individuals with serious mental illness maintain their health and avoid hospitalization.
Intake Process: Adults are referred through the Onondaga County Single Point of Access, while children are referred via the Onondaga County ACCESS Team.
Cost: There are no fees associated with the services.
