This program addresses a range of issues such as falls, balance and mobility challenges, dizziness, incontinence, increasing memory loss, confusion, and disorientation. It also caters to individuals with multiple chronic illnesses, particularly when there are numerous medications or healthcare providers involved. Additional concerns include persistent mood, motivation, or sleep issues, chronic pain (like arthritis), difficulties with self-care, and general frailty. Services provided include thorough assessments of medical, functional, cognitive, and psychosocial challenges, assistance in managing health issues and social requirements related to aging, continuous medical follow-up and case management for dementia patients, as well as education, counseling, and support for families, caregivers, and professionals. The program works in collaboration with primary care physicians and other healthcare providers. Intake Procedure: Interested individuals can apply by calling or visiting in person. Fees: Charges depend on the specific service; please call for details.
