This initiative provides a variety of resources and support for older adults and their caregivers, which encompasses: information, referrals, and advocacy; case management; Long Term Care Options Counseling to assist consumers in making informed care decisions; benefits counseling and help with applications for public benefits like HEAP, Medicaid, EPIC, and the Medicare Savings Program. It also includes health insurance counseling, Medicare 101 workshops, transportation information and referrals, legal assistance, caregiver counseling, and support. Additionally, it offers a senior housing guide, nutrition services such as home-delivered meals and congregate dining options, nutrition counseling, and a farmers market coupon program. Community-based support services are also available, including homemaker/personal care services, housekeeping/chore services, in-home assessments, referrals for grab bars, rails, ramps, personal emergency response systems, respite care, and health promotion programs like Project Lifesaver and Matter of Balance. Intake is available by calling the office for assistance, and while donations for meals and services are appreciated, EISEP operates on a sliding fee scale.
