The Welcome Orange Geriatric Initiative (WOGI) Gatekeeper Program serves as a mobile outreach effort focused on proactively spotting older adults in the community who are at risk and not engaged with the service delivery system. It aids seniors by assessing their signs and symptoms through in-home evaluations, facilitating access to a range of supportive services. This initiative aims to help at-risk seniors remain in their homes and avoid unnecessary out-of-home placements by addressing their unmet needs. It's important to note that this program is not intended for services offered by licensed outpatient programs or other active OMH funded initiatives.
