OnTrackNY is a progressive outpatient treatment initiative tailored for adolescents and young adults dealing with mental health concerns. This program assists participants in reaching their objectives related to education, employment, and personal relationships. Enrollees will benefit from a comprehensive support team, including a Psychiatrist, Nurse, Therapist, Peer Specialist, and Education and Employment Specialist.
Intake Process: Submission of an application is necessary.
Fees: Clients are encouraged to contact the agency for specific fee information.
