This program delivers structured coordination and management of medical, mental health, substance abuse, and social service requirements for Medicaid individuals dealing with chronic conditions. The Care Manager aids individuals in navigating their care by facilitating communication with healthcare providers, guiding them through the intricate healthcare system, and connecting them to resources for affordable housing, food, clothing, childcare, and transportation. Fees: There is no charge for individuals who are enrolled in Medicaid. Additionally, services for non-Medicaid individuals are also available at no cost; please call for further information.

- To qualify, individuals must meet four criteria: 1 - Be a resident of Orleans County 2 - Be enrolled in Medicaid* (including those dually enrolled in Medicaid/Medicare) 3 - Have two or more chronic health conditions (like asthma, diabetes, mental health issues, or substance use disorders), a significant mental illness, or be living with HIV/AIDS 4 - Present significant behavioral, medical, and/or social risk factors that can be addressed through Care Management.
