Northern Rivers Family Of Services

Children from birth through age 21 who are enrolled in Medicaid and meet at least one of the following criteria: 1) Have a minimum of two chronic conditions, 2) Have a diagnosed Severe Mental Illness (SMI) such as Severe Emotional Disturbance (SED), or 3) Are living with HIV. Additionally, pending CMS approval, criteria may expand to include those experiencing trauma and at risk for developing another chronic condition.

Intake Procedure: Requires a referral from local DSS.

Fees: No cost involved.
