Putnam Independent Living Services provides support to Medicaid-eligible individuals with Traumatic Brain Injury, facilitating their transition from nursing homes and long-term care facilities into the community, or helping them obtain services to prevent nursing home placement. We assist individuals in navigating the eligibility process, choosing a service coordination agency, and accessing various community benefit programs and services. Putnam Independent Living Services serves as the regional center for the Hudson Valley, catering to residents in Westchester, Rockland, Orange, Sullivan, Putnam, Dutchess, and Ulster Counties.
