This program assists youth who are transitioning home or are at risk of requiring intensive services, such as inpatient or residential care. It provides support in accessing services and navigating systems related to mental health, substance abuse, and developmental disabilities.
Intake Procedure: A referral packet is submitted to the SPOA Coordinator, and records are assessed to confirm eligibility; additional information can be obtained via phone.
Fees: There are no charges for these services.
