This program offers support for individuals with coordination needs, including goal setting, health promotion, transitional care, referrals to community and social services, patient and family support, and advocacy.

Cost: Free of charge.

- To qualify, individuals must be enrolled in Medicaid or eligible for it, have a diagnosis of two or more chronic physical conditions, HIV/AIDS, or a serious mental illness, and face significant behavioral, medical, or social risks.
