This initiative delivers integrated and supportive housing for individuals with disabilities who are at risk of homelessness. It accommodates Medicaid recipients aged 55 and above who need help with daily living activities, individuals 18 and older who qualify for services from the NYS Office for People with Developmental Disabilities (OPWDD), and disabled military veterans. On-site staff are available to advocate for residents, link them to community services, provide training in independent living skills, and organize social and recreational activities.
Fees: No fee.
