This initiative provides stable housing for low-income or homeless individuals affected by HIV/AIDS. It offers a range of supportive services, such as case management, transportation, advocacy, referrals to medical, mental health, and substance abuse services, crisis intervention, permanency planning, and education on HIV. It is associated with Transitional Services of New York for Long Island, Inc.
Intake Procedure: Clients are encouraged to call the agency for further information.
Fees: 30% of adjusted income.
