This initiative, funded by a grant from NYSDOH, is focused on improving the health of women between the ages of 12 and 44, along with their families. The Suffolk County MICHC project specifically targets ten high-risk zip codes in Western Suffolk County: Amityville, 11701; Babylon, 11702; Bay Shore, 11706; Brentwood, 11717; Central Islip, 11722; Commack, 11725; Copiague, 11726; Deer Park, 11729; Lindenhurst, 11757; North Babylon, 11703; West Babylon, 11704; and Wyandanch, 11798. These areas showcase significant racial, ethnic, and economic disparities. The Suffolk County MICHC employs various strategies and activities to address maternal and infant health behaviors throughout women's lives, aiming to ensure optimal health before, during, and after pregnancy. Services include education, case coordination, and home visits for pregnant and parenting women, as well as linking low-income women, teens, and families to health and social services to enhance care utilization and continuity. Both group and individual educational sessions are provided. Additionally, the MICHC initiative will establish a collaborative network of local health, social service, and community providers to improve service access and identify service gaps. This Collaborative also focuses on recognizing and overcoming barriers to health for women of child-bearing age.
