This program acts as an entry point for long-term services aimed at older adults and individuals of all ages with disabilities. The services offered encompass personal care assistance (such as meal preparation, housekeeping, bathing, transportation, home safety, accessibility, and managing household finances), as well as education, counseling, behavioral health and developmental support, health and wellness services, insurance information, and options counseling.
Intake Procedure: The agency encourages clients to call for more information.
