The Open Door Program offers assistance to individuals residing in nursing homes and intermediate care facilities, helping them obtain tailored home and community-based services. The Transition Center is dedicated to guiding these individuals in accessing local services and supports. Transition specialists are available at Independent Living Centers across New York State, with the Suffolk Independent Living Organization hosting the Open Doors Transition Center and Peer Program for Long Island. These specialists collaborate with clients and their families to determine necessary services and facilitate their acquisition. The Peer Program consists of individuals who have previously lived in nursing homes and intermediate care facilities and are now receiving local support. They visit these facilities to share their experiences and encourage residents considering a move to the community.
Intake Procedure: Clients are encouraged to call the agency for more information.
Fees: There are no fees associated with this program.
