This program delivers short-term, intensive support to individuals living with HIV, targeting both those who are newly diagnosed and those who have been diagnosed but are not currently receiving care, aiming to effectively connect them with medical services and case management. Additionally, free rapid HIV testing is provided. Clients of SPARC Consortium member agencies can access respite and transportation services through the program. The Women’s Care Coordinators at SPARC focus on ensuring clients remain engaged in care and are connected to other essential community services. Furthermore, SPARC's nutritionist conducts thorough assessments and offers both individual and group educational sessions.
