This program offers customized assistance for pregnant individuals and families with children aged 5 and younger, delivered through home visits. Services include child development guidance, parenting support, specialized assistance for pregnant and parenting teens, and connections to community resources to fulfill basic needs and achieve family objectives.
Intake Procedure: Referrals can be made by agencies or through self-referral.
Fees: There are no charges for these services.
