This service offers transportation to ambulatory adults residing in the Towns of Delphi Falls, Fabius-Pompey, Lafayette, and Tully who are unable to drive themselves or secure other means of transportation for their medical and dental appointments on Tuesdays and Thursdays.
Intake Procedure: Individuals should call at least 3 days before their scheduled medical appointment at 315-696-8659; the dispatcher is available Monday through Thursday from 9 AM to 4 PM.
Fees: Clients must cover parking fees and are encouraged to bring their handicapped parking tag if applicable.
