The 'It’s About You' initiative is dedicated to enhancing maternal and infant health outcomes by adopting a life course perspective, targeting clients at various stages including preconception, prenatal/postpartum, and interconception. It seeks to empower both women and men to take charge of their health by providing health education resources, connecting them with community support services, and engaging in advocacy efforts. Key elements of this initiative include the involvement of Community Health Workers, home visits, risk assessments, personalized service plans, referrals, preventive screenings, and health coordination.
