This program delivers a range of home care services for Medicaid recipients who are elderly, infirmed, or disabled. Services include personal care, case and care management, personal emergency response systems, and long-term home care for those eligible for nursing homes but preferring to remain at home. All services are ordered by a physician and overseen by a registered nurse, with care provided by licensed professionals contracted and monitored by the Department of Social Services.
