This program offers a range of home care services for Medicaid recipients who are elderly, infirmed, or disabled. Services include personal care, case management, emergency response devices, and long-term home care for those eligible for nursing home care but prefer to remain at home. All services are ordered by a physician and overseen by a registered nurse, with care provided by licensed professionals who are contracted and monitored by the Department of Social Services.
