This community-oriented initiative assists individuals dealing with chronic physical, mental health, or addiction issues. Care Coordinators play a vital role in facilitating connections and advocating for individuals to help them attain their objectives and thrive within the community. They can provide referrals for various services, including treatment, primary healthcare, specialists, housing, transportation, education, vocational training, and peer support, among others.
Intake Procedure: You can make a referral directly online at https://www.hhuny.org/members/make-a-referral/ or by fax at 585-613-7670. Referrals may also be directed to the Wyoming County Single Point of Access (SPOA) through the Wyoming County Mental Health Department.
Fees: No cost.
