This program offers a centralized intake system for both adults and children experiencing serious behavioral health issues, including those with medical and/or social risk factors. There are three categories of SPOAs: Children's, Adult Case/Care Management, and Adult Housing. The SPOA process focuses on identifying individuals who require services the most and oversees their access to and use of these services.
Intake Process: A referral form must be completed. Visit the website to obtain the form, selecting either Adult or Children SPOA on the left side. Scroll down to find the link for the (Adult) A-SPOA referral form or the (Child) C-SPOA Application. Fees: There is no charge for the intake process.
