Provides a variety of support services for individuals facing intellectual and/or developmental disabilities. These services may encompass case management, personalized support plans for individuals and families, access to brokerage services for transportation, in-home assistance or residential placement, as well as employment and vocational resources.
Intake Procedure: Contact to obtain an application or arrange an appointment.
