Counselors offer insights into support strategies, action plans, and options available within the community. Options Counseling may serve as an independent service or can transition into Older Americans Act Case Management (OAA), which supports individuals needing ongoing assistance to maintain their independence. Care coordinators play a role in problem-solving, advocacy, and connecting participants with resources and community support to improve their quality of life and facilitate future planning.
