Community Action Program Of East Central Oregon
Community Resource
Acerca de este recurso
Options counselors provide support to individuals in making educated decisions about long-term care. They evaluate personal strengths, needs, preferences, and specific situations. Counselors offer insights into support strategies, action plans, and options available within the community. Options Counseling can function as an independent service for participants or may transition into Older Americans Act Case Management (OAA), which assists those needing ongoing support to maintain independence. Care coordinators help with problem-solving, advocacy, and connecting participants to resources and community support to improve quality of life and future planning for later years.
