The services provided include routine physical exams, sports physicals, treatment for illnesses and injuries, vision and blood pressure checks, immunizations, mental health support, age-appropriate reproductive health services, routine laboratory tests, management of prescription medications, and educational initiatives on health, fitness, and nutrition.
Intake Procedure: Schedule an appointment by calling.
Travel/Location: The facility is situated next to the tennis courts at Forest Grove High School.
Fees: They accept OHP and certain private insurance plans, with sliding scale fees available. No student will be denied services due to inability to pay.
