Nutrition Support is a service under the Oregon Health Plan aimed at addressing health-related social needs (HRSN). It assists members with specific health conditions that can benefit from nutritional interventions. This service encompasses several components: Medically Tailored Meals (MTMs) which may include fresh or frozen foods, providing up to three meals daily for a period of six months, delivered to the member's home or a private residence, including shelters that do not offer meals. Nutrition Education involves evidence-based guidance on making healthy food choices and adopting beneficial food-related behaviors, tailored to individual dietary preferences and cultural considerations. Pantry stocking offers a portion of the recommended daily food intake for up to six months, with options for fresh, frozen, dried, pureed, or canned foods delivered as a grocery box or via a pre-loaded card for use at select grocery stores. Additionally, a benefit for fresh fruits and vegetables is available for six months, including various forms such as canned, frozen, or dried. Qualifying members receive this benefit similarly as a grocery box or pre-loaded card.
To initiate the process, individuals can call, email, or visit the website for further details. Following an initial screening, a referral is made by the Coordinated Care Organization (CCO) to a registered dietician for an assessment to validate the need for MTMs and to ensure that the MTM care plan objectives are met.
There are no associated fees.
