This program delivers extensive care management through a dedicated lead care manager, ensuring the coordination of all health-related services. It encompasses physical health, mental health, dental care, and social services. The initiative targets specific adult groups, including those at risk for avoidable hospitalizations or emergency department visits, individuals facing birth equity issues, those experiencing homelessness, community residents at risk of long-term care institutionalization, nursing facility residents transitioning back to the community, and individuals with serious mental health or substance use disorder needs. Additionally, it addresses youth populations at risk for avoidable hospitalizations or emergency department visits, homeless families or unaccompanied children and youth, and those with serious mental health or substance use disorder needs, particularly children with high adverse childhood experiences (ACEs) scores.

Intake Procedure: Applications can be submitted online or assistance can be requested via phone.

Fees: There are no associated fees.
