This program links low-income, uninsured individuals to donated specialty, dental, and behavioral health services, along with case management assistance.
Intake Process: The procedure varies based on the required services. A brief, confidential phone screening will be performed by a member of the care coordination team. After this, you will be directed to a suitable provider who has been vetted to address your specific needs.
Cost: There are no fees involved.
