Trillium Community Health Plan

Nutrition Support is a health-related social need (HRSN) service offered by the Oregon Health Plan, designed to assist members whose health-related conditions can be improved through nutritional support. This service encompasses several components: Medically Tailored Meals (MTMs), which may include fresh or frozen meals delivered to the member's home for up to three meals per day over a period of six months; Nutrition Education that provides evidence-based strategies for making healthy food choices and culturally appropriate dietary guidance; and Pantry Stocking, which supplies a portion of the recommended daily food intake for up to six months, available in various forms such as fresh, frozen, dried, pureed, or canned foods. Eligible members receive these benefits through grocery boxes or pre-loaded cards for specific grocery stores. Additionally, there is a provision for fresh fruits and vegetables for up to six months, also delivered as grocery boxes or pre-loaded cards.
