Delivers vocational rehabilitation services to eligible injured workers. Applications can be obtained from the hospital or doctor's office where the initial treatment was provided and must be submitted within one year from the injury date.
Intake Procedure: Interested individuals should call or visit in person to register.
Fees: There are no associated costs.
