This program provides vocational rehabilitation services to eligible injured workers. To apply, individuals must obtain an application from the hospital or doctor's office where their treatment began. Applications must be submitted within one year of the injury.
Intake Procedure: Interested individuals can sign up by calling or visiting in person.
Fees: There are no fees associated with this service.
