This program offers paid leave for residents of Washington state when they are unable to work due to a serious health issue, need to care for a family member, welcome a new child, or handle certain military-related situations.

Application Process: You can apply online through the website or request a paper application by phone. The Certification of a Serious Health Condition form can be sent to your healthcare provider via email and returned to you, after which you can upload it to your Paid Leave account. Electronic signatures are permitted. Once your application is submitted, the staff of the Paid Family and Medical Leave program will review it to: Verify that you have worked sufficient hours to qualify, confirm that your situation is eligible for paid leave, document the duration of leave certified by your medical provider (for your own medical leave or for caring for a family member), and assess the amount of pay you may receive during your leave.

Cost: There are no fees associated with this program.

To qualify, applicants must have worked a minimum of 820 hours in the last year, averaging about 16 hours per week in Washington state, and possess a certification of a serious health condition. Eligible family members for whom residents can take paid leave include: spouses and domestic partners, children (biological, adopted, foster, or stepchildren), parents and legal guardians (or in-laws), siblings, grandchildren, and grandparents (or in-laws).
