This service issues death certificates for individuals who passed away in Davis County from 1973 onward. For deaths that occurred prior to August 1, 2006, certificates must be requested from the county where the death took place. Certificates for deaths before 1973 can be acquired from the Utah State Office of Vital Records.
Intake Procedure: Applications can be submitted online at: https://daviscountyutah.gov/health/health-services/birth-death-certificates/vital-records
