The initiative seeks to enhance the health and quality of life for children with special needs and their families by supplying hearing aids.
Application Process: Submit your application online at: https://familyhealth.utah.gov/cshcn/chap/

- Eligible participants must be under 18 years old; the program is designed for Utah children diagnosed with hearing loss who cannot obtain hearing aids through Medicaid or private insurance, and whose family income is at or below 300% of the federal poverty level.
