This program provides services that enable families to make necessary physical modifications to their homes to support the health and wellbeing of individuals.

- Applicants must have a documented disability; eligible conditions include intellectual disabilities or related developmental disorders (such as autism, Down syndrome, genetic disorders), Acquired Brain Injury (ABI), or physical disabilities resulting in the loss of function in at least two limbs. Additionally, applicants must reside in Utah, be U.S. citizens or legal permanent residents, and hold a Medicaid Waiver.
