This initiative provides a comprehensive directory of Diabetes Self-Management, Education, and Support Services (DSMES) tailored for individuals living with diabetes. The program aims to educate participants on maintaining a healthy diet and active lifestyle, regularly monitoring blood sugar levels, understanding diabetes medications, managing the emotional aspects of diabetes, and minimizing the risk of additional health issues. It also includes workshops covering various topics related to diabetes and other chronic illnesses.
