- Eligibility for the program requires individuals to have a serious and ongoing mental illness, experience literal or intermittent homelessness, show a commitment to addressing mental health issues and substance use, and either be uninsured or covered by Medicare or Medicaid. After meeting these criteria, a comprehensive intake assessment will be conducted.
