- To qualify, patients must be insured and have coverage for the medication they need assistance with, possess a physician-confirmed diagnosis of the relevant disease, reside and receive treatment in the U.S., and have an income below the fund's guidelines, which are typically set at 300% or 400% of the Federal Poverty Guideline (FPG), taking into account the Cost of Living Index (COLI) and household size. Patients will be notified immediately after applying if they meet the criteria for assistance.
