- - Must be a U.S. Citizen or a permanent resident.- Must have a confirmed diagnosis of a chronic or life-threatening illness or require screening services for disease symptoms.- Must be undergoing active treatment, have received treatment in the last 6 months, or be starting treatment within the next 60 days at a facility in the U.S. or U.S. Territories.
