This program provides vocational rehabilitation services for eligible injured workers. Applications can be obtained from the hospital or doctor's office where the initial treatment was provided and must be submitted within one year from the date of the injury.
Intake Procedure: You can sign up by calling or visiting in person.
Fees: There are no fees associated with this service.
