This program provides vocational rehabilitation services for eligible injured workers. Applications can be obtained from the hospital or doctor's office where the initial treatment was given and must be submitted within one year from the date of the injury.
Intake Procedure: Interested individuals should call or visit in person to enroll.
Fees: There are no associated costs.
