Open Doors For Multicultural Families

This program assists older adults and individuals with disabilities in making informed decisions regarding supports and services to promote independent living in the community.

Intake Procedure: To make a referral, call the service. Referrals are accepted from Community Living Connections specialists and individuals calling on behalf of clients. CLC specialists must provide clients the option to call themselves and cannot make calls on behalf of third parties. They may only call for clients with direct consent. If there is no answer, leave a voicemail containing as much client information as possible, including: - Full name - Phone number - Address - Date of birth - Reasons for needing Options Counseling - Description of required services. Third parties can refer clients by calling and providing the necessary information.
