Delivers extensive primary care, case management, and home care services for individuals aged 55 and older, in addition to a day health center and the supply of durable medical equipment.
Intake Procedure: Contact the intake representative to undergo a phone screening for eligibility. A tour of the facility or a home visit can be arranged to discuss the program in more detail. Should the applicant decide to enroll, the intake representative will help with the necessary paperwork.
Fees: Enrollment requires being on WA Apple Health (Medicaid), and individuals may also have Medicare/Medicaid.

- Applicants must be 55 years or older, reside in most areas of King or Snohomish County, and be certified as functionally eligible for PACE (or COPES) by DSHS, primarily serving those with WA Apple Health (Medicaid) or Medicare/Medicaid.
