This program provides outreach and intensive case management for homeless, low-functioning adults facing severe mental health challenges, and it runs a specialized drop-in center for these individuals. Typically, a referral is needed from a caseworker or a DESC outreach worker.
Intake Procedure: Initial engagement occurs with potential clients on the streets, at service centers, hospitals, and other locations. Key outreach sites include DESC, Angeline's Day Center, Women's Referral Center, Hammond House, and Noel House. Referrals are welcome from service professionals, paraprofessionals, and community members, with referral calls also accepted.
Fees: There are no fees for services.

- Individuals facing homelessness in Seattle who are extremely vulnerable, lack support connections, and are unable or unwilling to participate in conventional service models are eligible, provided they eventually qualify for Medicaid.
