For other household members: include all individuals living in the home along with their relationships and birthdates. Property address and mailing address, duration of home ownership and occupancy, phone numbers, and income details (including all sources of income and gross monthly amounts) are also required. For those seeking loans: details about the mortgage company, monthly mortgage payment, remaining mortgage balance, and current status of payments should be provided. Information regarding any other loans, judgments, or liens must also be disclosed. For mobile home residents in parks, information on space rent and any purchasing agreements must be included. Lastly, details about assets such as bank accounts (including bank names and balances), investments (stocks, bonds, mutual funds, etc.), and any other real estate owned should be prepared.
