Eligible veterans can access home and community-based services to help them remain in their homes and communities, provided they are enrolled with the VA Puget Sound Healthcare System.
Intake Process: Contact for further details. A referral from the VDHS liaison at the VA Puget Sound Health Care System is required.

- Veterans must be enrolled for primary care at a VA Puget Sound Health Care System (VA PSHCS) facility, have a VA primary care team, and fulfill the VA's criteria for home and community-based services. This includes having three or more dependencies in activities of daily living (ADL), significant cognitive impairment, receiving hospice care, or meeting other specific conditions.
