This initiative provides complimentary case management for mothers dealing with substance abuse issues who are either pregnant or within 12 months postpartum. Once they join the program, participants are guaranteed support for a duration of three years.
Intake Procedure: To start the referral process, visit the website. Typically, mothers are referred by a community provider.
Fees: There are no charges for the services.

- Eligible participants include women who are pregnant or within 12 months postpartum (or 24 months if space permits), self-report substance use during their current pregnancy, and are not effectively linked to community resources. The program also welcomes mothers with a child diagnosed with Fetal Alcohol Spectrum Disorder who are currently using substances and are of childbearing age.
