This program addresses the emotional, spiritual, and physical requirements of individuals facing a terminal illness with a prognosis of six months or less, as assessed by their doctor. Patients and their families collaborate with an interdisciplinary care team.
Intake Procedure: Contact the Referral Center at 360-814-5550.
Fees: Medicare, Medicaid, and most private insurance plans cover the services. Eligible patients can receive care regardless of their financial situation.

- Two doctors must confirm that the patient is expected to live six months or less if the illness follows its natural progression.
